Habilitation Specialist Full time, Hourly Position
|

Description |
Assist individuals referred from CCBMR/DD to meet their basic living skill goals by completing the following. Shape personal and social behaviors of clients in a pleasant, therapeutic environment. Prepare and manage all individual case files and write detailed and accurate case notes on activities for each assigned client. Write monthly reports and conduct quarterly reviews regarding client’s progress. Assist the Program Supervisor with developing and implementing long and short-term individual support plans based on the client’s needs and goals. Attend annual and/or quarterly ISP meetings as requested. Responsible for keeping the support administrator informed of the client’s progress and acts as a liaison to the referring support administrator, family, guardians and other individuals actively involved in the client’s life. Supervise lunch and break activities, aid in using the restroom, feeding, and other activities as necessary. Will ensure that the client is kept clean and unsoiled, and if needed will assist them in clean up and changing of clothes. Recommend and implement program improvements and continue professional development through workshops and seminars.

Qualifications |
Bachelor’s degree (B.A.) from four-year college or university in a related field or two years related experience and/or training with the MR/DD population. Proven experience working with MR/DD population is required.

Job Coach Part-Time, Intermittent One Position open in Cleveland & one position in Painesville
|

Description |
Provide a variety of job coaching services for clients including but not limited to developing appropriate job keeping behaviors, task analysis, development of skills to complete specific job tasks, development of natural supports and the identification and implementation of reasonable accommodations at the job site. Assist clients in the development of an individualized success plan to aid them in achieving measurable objectives. Act as an advocate for the client on the job site and provide employers with information on effectively working with individuals with disabilities. Responsible for providing clients with travel training services to aid them in their vocational success. Job Coach services extend beyond normal agency business hours, 8:00-4:30, and on weekends as necessitated per assignment.

Qualifications |
Associate’s degree (A.A.) or equivalent from a two-year college or technical school; or six months to one year related experience and/or training; or equivalent combination of education and experience. Must have ability to work independently. Basic computer skills in Microsoft Word and Excel are required.
